Actually, Carmacks first engines (Wolf3D, Doom) before Quake did not have much to do with actual 3D. Although they were great performance-wise they didn't have anything to do with polygon texturing or such.
Wolf3D was just a simple raycasting engine and Doom developed a little over that to include the Z-factor (different heights). Remember that there was no possibility for bridges or overlapping sections of any kind.

The main no-real-3D thing with the Doom engine was that walls were always vertical, even when looking up or down.
