Initially I had a problem installing the game from the splash screen. I tried it again after rebooting and the install started. Afterwards I ran the SS2Tool v3 to patch and prep the game for modding as well as setting widescreen 1920x1200 res. Next I installed SHTUP beta 6 and Rebirth. In order to actually load the game I have to run the .exe in XP SP3 compatibility mode, otherwise the game doesn't load and any other compatibility settings don't work.
Finally, at the main menu I opened task manager and set the games affinity to a single core - CPU 1 (I have 8 cores), CPU 0 doesn't work.
The game runs fine except the main menu flickers with thick black vertical lines and the game has extreme screen tearing. There's no vertical sync in the options so I tried forcing it on in ATI control panel - this makes no difference. Any setting to the vertical refresh makes no change to the game whatsoever.
So, how do I cure the flickering main menu and how do I turn on v-sync?

As you noticed the game has no v-sync option so you were right setting it in your graphics card settings. If that doesn't work then you either did something wrong or v-sync isn't the problem. You could try disabling DDFix via ddfixgui.exe.

I've tried disabling VideoFix, changed RefreshRate to 60, 70 and 100, set z-buffer to 0 but this causes the game to not load, saying my hardware is unsupported. I've still got the black lines going up and down the main menu (like some crazy ZX Spectrum load), the main menu sometimes overlaps the game so I can't see where I'm going and the screen tearing is still present.
