I personally spent most of my time in Point Lookout sneaking around. 100 Sneak skill + Chinese stealth suit + silenced Perforator = few problems.
So stealth is effective? I'm considering some add-ons and giving FO3 another chance. If stealth is viable and now there are Stalker like mods, I'm getting much more interested.
Sounds like Broken Steel and Point Lookout are the ones that seem most intriguing.

Stealth in FO3 mainly serves two purposes A. ambushing enemies, giving you a chance of scoring a critical. B. Sneaking around and stealing shit, pickpocketing, reverse pickpocketing (Eg planting a grenade or mine on somebody, upgrading their equipment), etc.
When crouched you automatically go into stealth mode and an alert status is displayed on screen. These statuses are:- Hidden, Caution and Danger.
Stealth kills with energy weaponry never get old. A head shot whilst in VATS for a character with 100 Energy stat almost always results in the target being liquidified into a pile of ash (with laser weapons) or pool of goo (plasma weapons).

Better yet, using VATS to vaporize multiple enemies in one go. With enough action points, its satisfying to use VATS only once and take out 3 or 4 raiders easy.
Anyways, eva explained stealth pretty well. The Chinese Stealth suit I mentioned comes with the OA DLC, while the silenced rifle comes with Broken Steel. Combined they're perfect. Even if you don't kill the enemy with the first shot, they can't tell where the shots were fired from AND you're completely invisible (while crouched, the stealth suit acts like a Stealth Boy).

Actually the silenced Assault Rifle (the Perforator I believe. Although that might be the name of a unique version that you get as a quest reward for collecting a certain number of Ingots, I forget.) comes with The Pitt.

Right, my mistake. That said, the Perforator is the unique version. The standard is the Infiltrator.
