i'd agree with random activity, as well as anything unknown... think of kittens, when they see ANYTHING that they've never before seen, they jump and run and attack it and shit. dogs are the same way.
more specifically, i'd say movement. spiders are creepy because of how they move, as are snakes and asians.

In terms of human-like creatures it's most certainly something that is just a bit... off. This is how Lynch does it, he has these almost normal but somehow just wrong people with some weird background music to cause further disjointedness. See the Mystery Man in Lost Highway for a good example, or the guy in the diner in Mulholland Drive. We see it in the high speed oscillations superimposed on the normal movement in Ju-On and Jacob's Ladder.
It's a shame that we are becoming over-exposed to this kind of stuff though. It's in so many films now and it's having less impact on me at any rate. I miss that shiver it used to give me. I CRAVE STIMULATION NO STITCH NOT THAT UNDERSEA FREAK THOUGH IT'S TOO MUCH.

The design of freaky creatures needs to update in sync with the tolerance of today's generations. Imagine what people decades ago would think when seeing horror scenes of today. :p
