Zylon and others are right... Gameplay is more important than realism. Yet it would be great if anyone could find out how to distort vision when certain conditions are fulfilled. For example: Garrett eats poisoned apple and sees "blurry" world until certain antidote is found.
But perhaps it is common knowledge already? We had something like that in the Cradle...

It would be possible to do what ascottk is suggesting without it being annoying. I agree that it's a bad idea to tint the whole screen red and jerk the controls around and stuff, but it doesn't need to be that extreme. A subtle blur and/or red tinge around the edges of the screen, plus sound effects such as Garrett breathing heavy when he is damaged. This way it wouldn't really impede gameplay and would be more like a part of the GUI in a way.
