Whatever. The game is what it is and ever has been. Keyholing that particular door is mostly safe, I think. At least safer than going through all of the towers.

The last ladder on the outside in the chamber with the Quinus horn is probably one of the most dangerous places in the whole game. :p

Yes that ladder can be tricky. But if I climb to the top of it so that I am now standing on it, turn around and press W a moment before space, the jump always seems to be made, if I jump with the section I am jumping to roughly centre screen before starting the jump.
I have now TMed the game in normal and hard, and currently working on expert, and with many failed attempts besides, I can say that for me Assassins tailing remains the headache. Not helped by the fact that they can take seemingly random routes through the City, and the sound propagation is not great in some parts as you cannot hear them walking when you should. So you might hear no footsteps once they go around a corner and not know whether this is because they are standing still looking behind them, in which case it is dangerous to lean around and look, or are getting away from you if you do not check.
I would happily swap that section for a dozen trips around the outside ledge of the building with the Fire Talisman in Lost City, or the section you have to jump from three ropes to get across a lava pit :)
Although the games room in Thieves Guild can also be tricky to pull off without setting off a lot of guards. Oh well, at least there are plenty of places to run to there.

You're probably just following them too closely. They don't stop and look around just for fun but because the've heard you. If you do it right, they never search for you at all.

Guards won't hurt you if you stand on a table (unless they have a bow).

It seems that they will always stop in certain places no matter what I do. Even if I am not moving and they are around a corner I know that if I wait they will keep walking then stop in their regular spot even though I am still. Oh well, it has gotten easier.
