The best way to save old Miguel is to tell him to stay put once you start the level. Then go and finish it all on you're own, and go back and get him when you've cleared the place out. Since self-preservation is rarerly a virtue of video-game characters, this is probably the best strategy whenever you encounter "escort" missions in a game.
I think that if you make it out, he gives you (another) Medkit, but I can't remember. It was a bit disappointing.

I liked casually walking in front of the guy behind the bulletproof glass in that room near the entrance of UNATCO (inside) with the retinal scanner and hearing his reaction (then killing him because he attacks me).

:To get Paul to surivive, on your next playthru, kill the MiBs in his apartment, run out, and get killed by the troopers, but you have to die first, you will be transported to a jail cell. Free Miguel and if he says something about THEM BRINGING IN SOMEONE ELSE, or something like that, you have succeded in mkaing Paul survive.

Not entirely true. The sole trigger to whether Paul lives or dies is whether or not you sneak out the window of Paul's apartment. Unfortunately, this seems to glitch out sometimes and Paul will just disappear from the 'Ton. Yes, the quickest way to resolve the whole situation is to just eat a LAM as soon the MiBs appear, but it's far more satisifying (for me anyway) to fight all the way back to the Battery Park subway station.

I put a LAM right in the hallway when they are breaking down the door. Throw a couple of gas grenades, hit those augmentations and shoot them with some Tranc darts, and high tail it outta there. Never stay and fight, cause you will get your ass kicked. Which is the reason why is was so hard, but getting to Gunther is fun.

When I played through I didn't bother to take Miguel with me. That sort of thing makes me want to replay certain game segments.
Another long stretch of the game that could be quite different is the interval between [spoiler]Lebedev and the Templar Temple. You can save Lebedev by killing Navarre and Jacobson will cover for you. The question is, what is Gunther's reaction? Does he come to believe that you killed her, and is he as bent on killing you?[/spoiler]

The following is a description of how to save Paul and Miguel among other things including some game mechanics and such...
[spoiler]There are two ways to save Paul really. Either die in the hotel, or go out the front door. As long as you keep Paul in sight, he will not disappear. However, if you lose sight and sound of him for too long, he will vanish. This isn't really a glitch, a lot of AI are programmed to walk to a certain spot (usually near an exit) and disappear if you lose sight of them. This simulates them "leaving."
Paul is invincible at that point, so it's easy enough to follow him (keep him in sight) and let him kill everyone in the hotel. (He runs out of ammo, but has a knife anyway.) If you follow him all the way to the front door of the hotel and everyone is dead, the game still considers him dead if you exit out his window. If he disappears, but you still leave through the front door, he will be alive. It is beneficial to fight your way out of the hotel to the Battery Park station since you gain extra skill points.
Like people have said, to save Miguel, leave him in the cell, then clear out a path. Then have him follow you through the loading point. After that, let him wait while you clear out UNATCO HQ. Then have him follow you out. (Careful though, there are two doors that are one-way, if you leave him behind the door when it closes, you can't get him to follow you.) If you manage to have him follow you all the way to Jock's helicopter, he gives you a thanks before you take off. That's it. No medkit.[/spoiler]
