Could someone PLEASE update that thread? I actually submitted a document to one of the moderators, but he wasn't able to take care of it. My document, which by now is probably a bit outdated, would still be better than the current thread's info and layout. I can resend the document to another person who has moderator status if they are willing to update the thread. I would gladly do it myself, but I'm not a moderator.

Tip: Just post your document and ask for it to be stickied and the old FAQ to be deleted. Then you'll be the author and will be able to update it later yourself. It's the only way to ensure that you won't have the same problem in half a year again.
