If you count FMs as well, you'll have quite some amount of sex (see "Calendra's cistern"/"Calendra's legacy", for example), farting ("The 7th Crystal"), or feces production ("A Servant's Life"). And wasn't there a mission with someone emptying his bladder in recent years? If not, there's a TDM mission where a Hammerite uses a homeless or drunk as a toilet. Repeatedly. Yes, really.

The 'The rent is due - steal 2000 coins' objective has always confused me. I imagine in the Thief world you could buy at least a small house for that kind of money :p

Why would the "world's greatest thief" even be in a situation where they are struggling to pay rent, so frequently? Makes you wonder why the lesser thieves would even bother with such an unlucrative profession.

Most if not all the things mentioned in this thread are gameplay choices, not mistakes or inaccuracies.
Of course, there are lots of things which have to be unrealistic, in favour of playability.

I've never understood why Garrett moves so much more slowly when he's carrying his sword rather than having it in its sheath, simple laws of physics say he's still carrying the same weight so shouldn't make any difference to his speed.
And how can he jump from a really really high building and manage to grab a ledge by his fingertips without ripping his arms out of their sockets, medical miracle or bizarre physics again.
As for jumping into water from ridiculously high ledges and building, unless you are a professionally trained high jump diver, the chances of instant death is staggeringly high.

Re: the blackjack, when T2X used a hammer, I couldn't suspend my disbelief about each blow probably being lethal or close to it. I never got that feeling with a blackjack, if that says anything.

The general guard voice lines being independent of context.
"Must have been rats" makes no sense when the reaction is to a torch being doused. Or "Who'd be stupid enough to break in here" being said on the City streets, etc.
