Just a few... not a full list. Still, these are not enough to keep me from playing ;)
Missions that require a secret be found to complete the mission. If it is required, it is not a secret. Yes, it may be hard to find, but something that is a requisite to complete a mission is not a secret/bonus.
Having to walk around like a duck until my head bobs low enough to get through a hole. How about a crawl feature?
Remove those taps on your heels! At least we got booties in King’s Story.
Doors that open outward (toward you) for no reason other than to be annoying.
Lockers and other locked things that are locked, yet contain nothing, or contain a hammer.
Super-hyper-sensitive AI. Radioactive hearing. Superman sight. Hyper running speeds.
Excessive required tedious backtracking.
Levers and switches that activate something on the other side of the map (unless marked as such with a label/plaque).

There' an easy way to get in: When you get stuck, lean forwards into the hole and you'll get in immediately.

Lean in does not always work for me. I find it fails more often than it works. So, I just back up and bunny hop in a crouch for a while to get situated correctly. Lot's of quick saves if I am in trouble.
Oh and in agreement with many, I hate picking things up and “using” them immediately because the next item is not perfectly highlighted. Frob and use should never have been the same key. I “deselect” every item as I pick it up, just in case I activate or use something.
Small pet peeve - FM makers that do not make barrels and other objects solid. Some “famous” FMs allow one to cruise through barrels with ease. ;)

Strange, I never had any problems with leaning in the original missions. It does not work reliably in some fan missions. Unless you're playing a modded version and they've broken that there.

Yeah. YOu should check the recent scripting thread which has a solution for that problem.

There is a funny thing happening in some missions, both original and fan-made.
When you frob an open chest, it starts closing, but if you quickly look the other way, the lid stops halfway. Only after you regain a line of sight with the chest will it close fully.
I don't know how to feel about that :)
