Fallout 3 comes to mind, since the originals had no qualms about it, but Bethesda decided to change it. As for why a designer would think about it, they obviously have to take all possible player action into account as part of their job.

I have no intrinsic problem with what is described being in the game, it's all in how it's executed.
It could be groan-inducingly awkward and feel like some 12 year old boys designed the area (Fallout 2 suffered from a lot of that) or it could just come across as a matter of fact part of the world around Garrett, or even a good way of conveying a decadence which has infected the city. It's all in how it's done.
We shall see. I do find it unfortunate that they are using real curse words, even if only because I had a warm spot in my heart for the whole "taffer" thing. It also could potentially end up killing suspension of disbelief a bit.
