I'm playing my first go round on the 3rd difficulty.
Health does NOT regenerate, but if you're a halfway decent player you find more than enough food to make up for it. Upgrading your Quen sign to the shield mode is also enormously helpful as all you have to do is tank a few hits and you've got half your vitality back.

Also: I'm glad of that evolution. The best way i can describe is that we're getting to a style that combines the best aspects of Ubi-creed games with the best parts of Black Isle, or even Bioware before they got shitty.
