There's a readable that (almost) tells you what to do:
use snake blood and mercury on the heart AND find the scroll with the Death beyond Death spell

The snake blood and mercury are in the laboratory, posing as "standard flasks" (I don't remember what they were in the Thief II main game but they were some of the objects you put together to make other objects in the factory in Thief II).
Now, a couple possible bugs (I am playing "hard" difficulty, and playing the haunted hospital):
1) I was able to get the "get living patients to safety" objectives (both of them) to tick off by dropping them inside the hospital, just inside the door to the East Wing (the door protected by wood and unopenable). I don't think that was the intent - I certainly wouldn't consider "next to an unopenable door, near some baddies" safe.
2) After using the mercury and snake blood on the heart, the act of entering the "surgery room" with the mage, causing him to see me (near Melwin's room/secret area) ticks off the objective that I killed him. I assume that clearly was not the intent. Since I couldn't figure out how to use the "death beyond death" scroll (I had it, but using it on the mage didn't seem to work), I simply ran away from him and exited the hospital the way I entered to end the mission.

I believe that you can use the
death scroll at any time after destroying the heart to kill Nemazidah. I don't think you have to be near him for it to work.
In a church like room near the beginning of the mansion, along with a bunch of haunts and spiders. He alone is left standing if you blast the bunch with holy water.

As for the lab, there are two labs. One hidden with the heart, the other in plain sight. Chemicals are in the latter.
Actually you don't have to tackle the haunts.
They die along with Nemazidah. Interestingly, they fight him if you make them aware of each other.
