It changes any CREATURE into another random creature. So an orc could be turned into a rat. Or a rat into an orc.

its like a stick thingy and you get it by doing the shrine halfway between leyawiin and bravil on the west side

UESP has pretty much everything you need to know about anything.
Plus the following map, it's a bit more interactive, but not as good for printing as RyushiBlade's link. I prefer it however as you can set it to only show places of particular interst (e.g. shrines), so the map is a lot less clutered:
