The amusing bugs screenshot/video thread

I was replaying The Witcher, and saw a rather amusing bug which saw Gerlat look like someone with a sword in his side wearing a half-melted, too-large rubber Geralt mask - an odd bug that even persisted through quitting the game and reloading it.

So I thought, maybe it's time to make a thread for screenshots and videos of weird and amusing bugs.

The first thing that came to mind is the one time I completely fell out of the level in 'A Boy and his Blob in: The Rescue of Princess Blobette':

I had a Gameboy before I had a Nintendo or PC, and I can tell you right now that YOU ARE WRONG!

It was capable of at least two more shades of green that aren't being shown off in that shot.

I noticed something weird going on with Jesh's arms in AC: Unity once when he tried to pick a lock.
