I remember Temple of the Tides. It was very well done, I ran back and forth several times before figuring out that it teleported you in the center of the hallway.

I have wondered about making a teleport in a wide hallway rather than a narrow one. Maybe 5 volumes across the opening. 1,2,3,4,5 teleport to a,b,c,d,e.
