The "requested screen mode cannot be set" is indicative of a faulty resolution. You most likely don't have a proper resolution set in the cam.cfg. Open up cam.cfg and make sure it's 1024 768
Also check the ddfix.ini to see if there's some resolution lines in there. Remember to always change desired resolution with cam.cfg with the latest version of ddfix rather than trying to change it in-game.

It is. The line reads "game_screen_size 1024 768"

After having the game resolution set to 1024 x 768, did you try leaving safe mode and playing normally? It seems like you've tested in safe mode more than you have normal, is that correct?
I can't really tell what's wrong, sorry.
Can you open ddfix.ini with notepad and see if there's any resolution lines in there? (gWidth gHeight)

I have tried playing normally, from launching the .exe. Under all circumstances I've tried (vanilla game from GOG, Tafferpatcher 100 with various settings enabled, tafferpatcher 103 with various settings enabled, I get the same crash to desktop when I continue past the objectives screen.
There are no resolution lines including gwidth/gheight in ddfix.ini; only cam.cfg There are a lot of other graphics settings in that file; could any of those help?

Are you on a dual-core or greater system? If so, is Multicorefix=1 set in ddfix.ini?
Is videofix set to 1 in ddfix.ini?
Can you also confirm that the game is only running on one core (Alt tab when at main menu of thief 2, open task manager, right click thief2.exe > affinity > CPU 0 should be the only one checked)

Multicorefix and Videofix are both set to 1. I tried changing them to 0; same crash. CPU 0 is the only one checked when examining thief2.exe from a minimized game menu.

I would disable eyefinity and try to get it running on a single monitor in the 1024*768 resolution. as soon as that runs, we can think about what to do next.
