Mission is BROKEN.
Everytime I leave from where I start (click on door), I go to a place and after a couple seconds a skull appears failing the mission.

If you are dying/seeing mission failed right at the start, i could use more info, or a save to try and figure out what's happening. It's not a problem that has ever occurred during testing. Does it specify a particular objective that has failed?

It said I failed the "don't kill anyone" objective. I tinkered with the settings and got it to work. Thank you

If you search Duncan's bedroom, you should find a book detailing the vault security measures.
The electrified floor is turned off via a control panel in one of the towers.
