Ah, I have had this in the prison escape mission. I thought that I had corrupted the save when I sent Miguel on his way right when I met him. I dunno, the whole map seemed to be corrupted then (starting with the reprogrammable bot being hostile to me). When I left the mj12 facility, it crashed.
I reloaded to back in the Hotel then and replayed the whole escape.
