The Primary Goal is to scuttle the PCS Wall Cloud by destroying the five tri-weld seams with explosives and reversing the bilge-pumps to destabilize the ship and split the hull.
What I did is, I reversed the bilge-pumps, I think, before I destroyed the five tri-weld seams. I am positive I destroyed all five, and I know I went to the computer, logged in, and clicked the button to reverse the flow. I also flicked the switch next to the computer. However, I haven't completed the goal yet and it is still sitting there in my menu, so I don't know what to do.
Any ideas?

Pretty sure that if you've already reversed the flow, the ship should start breaking as soon as you destroy all welds.

Yes.
So, either you've flipped a flagtrigger back to false by hitting the 'reverse the flow' button twice, or you haven't hit all the welds. Or possibly even both.
You can check the flag state by enabling cheats, and typing "legend": select the edit flags button, and search for the flag named 'Bilge': if this is true, the bilge pumps are reversed. If false, click it and set it to true. If you don't THEN get the 'ship fucked' stuff going on, you've missed a weld.
Failing all that, and if you really can't be arsed, just set a flag called 'MS_ShipBreeched', though I suspect this will leave that particular goal uncompleted. You should still be able to carry on, though.
