Asking the thorough opinion of the TTLF forum members, are there any inconsistencies or plot holes that you have observed or noticed in the story of SS2?

I know of two things: the obvious example that has been mentioned many times: how can the Beta grove end up 67 trillion miles away in such short time, when it's not flying above light speed?
And another thing, the character of Malone, the OSA Spook, seems to die in two different ways. One log mentioned him being shot in the neck by a cyborg, another log mentions him being shot by something that 'unloads both barrels into his stomach'.

Harvester's first example is actually the only true plot hole that I can think of in SS2. Everything else can be adequately sorted out one way or another (i.e. the Malone paradox could be explained if there was another crew-member named Malone that doesn't appear in other logs, which is admittedly sort of a cop-out explanation, but it's also entirely plausible.
SS2 is one of the better examples of internal consistency that I can think of, and I'm a stickler for that sort of thing.

There are three Malone death logs: Killed by a cyborg, killed again by a shotgun hybrid and killed again when a resonator blows up on the Rickenbacker.
Apart from the more-than-one-Malone theory, Shevers's death guide suggests he may have been using the QBRMs, or mistaken for his identical twin McKay.
