No, I don't think so. You cannot pull out a gun without cocking it every time, and doing so will alert every NPC and mutant around. I mean, they can hear that at miles away, even in a noisy environment.
You can even make dogs attack you just by reloading your weapon!

Reloading makes a noise, but I think in video games it shouldn't be an "alerty" noise.

I prefer a few realism breakers like in games like the Splinter Cells, too much realism makes sneaking pretty much impossible. ;)
