lower_spawn_min <-100000000000>

in user.cfg

completely remove random spawns?

The best way to disable respawning is to not play. Try that.

Didn't you ask basically the same question in (http://www.ttlg.com/forums/showthread.php?t=125247) this thread? Just use 'Ultimate' Spawn Mod if the game is too hard for you.

Respawning is an integral part of the game. Why not play it as it was intended?

You cannot disable it completely without modifying the missions anyway.
