Might be worth a clean reinstall and then try DDFix. Also, let us know your processor, OS and video card specs, it might help.

running Win XP with Service pack 2 installed
I tried with a clean installation and the fix still doesn't work. neither does anything else, but yeah. I tried different drivers + clean install. no result.

I tried the GUI first. didn't work. tried with the patched exe available for download. didn't work. looked in the ddfix.ini also in cam.cfg and user.cfg (the latter for enabeling/disabeling safe texture manager). no luck. I moved on to the manual installation of the fix after reinstalling thief 2 again. ok, hexedited, set the resolution and... no luck again. I looked at the cam.cfg and ddfix.ini files, set some more things in them. tried different combinations, it had no effect. I then moved on the an eralier version of the fix and again, the same thing happened.
the fix did help me at least start up the game, not receive a crash or menu graphical glitches and see movies. (the intro movie played, so did the ones from the missions, since I tried starting a different mission assuming Running Interferance was bugged or something)

Do you have hardware settings on? Look for line game_hardware in cam.cfg, it must read game_hardware 1. Last time I installed T2 to a new computer it had this setting set to 0 for some reason. Game crashed instantly until I set it back to 1.

I remember seeing that setting on another obscure forum. I tried it. it's the same thing. Intro movie plays, menus work, I start the game, loading, boom, crash, not responding, the famous "thief2.exe has encountered a problem and needs to shut down". it made a difference once when I was playing around with the safe texture manager. I was actually able to see the first frame of the mission when it all blacked out and died in the usual manner.

You tried setting the processor affinity to one core instead of two? Might help.

Just in case you haven't tried it yet (and I realize you may have already)... have you tried running it in a couple of the compatibility modes?
Try more than one, as different people seem to have different results. For some reason, I have to run in Windows 2000 compatibility mode. Go figure.
Be sure to have DDFix installed and working (make sure you can get to the menu and play the intro video) when you try the different compatibility modes.
Bear with us here... it's just that at this point, with DDFix especially, it really seems like most people should be able to play Dark Engine games fine. We're in a very different situation now than we were just 10 months ago.
