The thief boards have a thread with a heavy number of responses with this topic and since Deus Ex is alot like Thief, surely we must have experienced at least once the stupidity of our actions.
Mine had me crackin'. I was on a roll because I manged to slice all the chinese troopers inside the ship with the Dragon's tooth, taking only minamal damage. I did not bother to save since I felt invincible. So off I went to toss a LAM into final section located at the bilge pumps. Threw it, laughed, missed the jump and fell right back onto the LAM. You can guess what happens next. :p
What about the rest of you?

Mine usually involve TNT crates....dropping because of those damn doors knocking them out of my hands, forgetting there is one right next to me when i shoot one to open a door, landing on them...thinking they are supply crates.....

Woah there, I think you'll find that _I_ am the worst player, particulary when I attempt to stealth it.
------
The Scene - Gas Station hostage of Tiffany Savage, night.
The chopper drops me off, and I jog towards the steep mud slope in front of me. I pull out my stealth pistol, flip on laser sight and look around. All is quiet. Gun drawn, I advance to an old sewer pipe. Gaining entry, I crawl along, then up a ladder into a small fenced area near the first half of the station. I quietly replace the sewer grill, and shift a rubbish bag out of the way to peer through the gap. An evil MJ12 soldier is standing a few feet ahead, on lookout detail for invaders.
Normally I'd make a small offering to Mictlantecuhtli, the Aztec God of Death and unload an entire clip into the troopers face neck and crotch, but I'm trying to stealth it instead. Activating my ACE super-jump aug, I leap 50 feet into the air, silently, cape flapping behind me in the evening wind, sunset light glinting off my armor etc., and come to land on the roof of the station. Dropping to a crouch, I see the dumbass troopers hanging hither and thither haven't spotted me. I crawl along the roof, towards the garage where poor tiger-faced Tiffany sits, imprisoned by a MiB Death Squad.
I am now above the garage. A raven caws in the night. Exo-suit clad ninja troops oil their twin arm-mounted laser cannons welded to their vat-bred flesh. The moment is nigh. I stand up.
"Hey! There's Denton! On the roof! Look!" - said ninja troops shout. Damn.
A swarm of metal insects fly towards me, intent on my annihilation. I step backwards, and fall through a skylight into the lap of a previously mentioned MiB. Glass flies everywhere, guns go off, lights ping out, people trip over corpses and a single dropped LAM, combined with the MiBs explosive neural death-chips causes the entire garage to explode, taking me and Tiffany with it.
-------------
Or, to put it another way, I fall off the roof right into a group of enemy troops. They shoot me.

Probably on the statue level. Once, for the hell of it, I went as high up as I could in the statue and cheated maxed out legs.
I [literally] took a running jump off the balcony and as I reached the peak of the jump, switched the aug off. As fun as the splat was satisfying. :thumb:
Then I decieded to create as large a pyramid of TNT crates as possible. I spawned about 70 tnt crates, the did the above paragraph again.
BOOM!!!!

TNT crates have repeatedly resulted in amusing expiration during my games. Opening a door only to have it bump into a TNT crate is always quite interesting, as is bashing apart a crate only to have the TNT balanced precariously on top smash into the ground at my feet with loud report.:erg:

I was LAM climbing on the Statue of Liberty, just because I could, trying to get as high as possible. At some point, the walls sloped in such a way as to become impossible to place a LAM and jump on it, so I was basically stuck 20 stories above the earth. Having nothing else to do, I decided to take pot shots with my sniper rifle at the security drone at the front entrance, which promptly returned fire, and one of its bullets hit the LAM...
What a mess...:p

In the Battery Park subway station, I discovered the hard way that triggering a scripted conversation will cause you to drop anything you're carrying. Including TNT crates.
"Thank you for resc--" BOOOOM!

I like to throwing plants around in UNATCO. On one visit I decided to drop one off the ledge by Manderley's office. This was at the point where there's a guard outside the office. Ooops.
The OTHER one was when I wasp iling stuff around Walton Simons when he stands in Anna and Gunther's office. I made the mistake of throwing a trophy cup on top of the pile....
I also recently found out that Jock can't be killed in the bar in much the same way.

LAM climbing: You need at least 2 LAMs or other grenades. I actually like gas grenades best for it, but it does not matter, really.
Crouch infront of a wall, glue one of the grenades at it, crouch forward, and you sit on top of it. Repeat, pause for a moment to retrieve the lower one, and continue.
I like it best with 3 grenades (<i>*glue, crouch, glue crouch, glue, crouch, retrieve, retrieve - repeat*</i>), as JC has VERY long arms.
I don't know which of my silly deaths I should pick to tell you here. ;)
However, here is a silly story (which actually did *not* end with my death though).
It was in Hong Kong, before I had retrieved the Dragon's Tooth. I decided to LAM climb up to the roof of the skyscraper next to the one with Max Chen on the top. Up on the top, I decided to have some mad fun.
So I attacked Max Chen a bit with my tranquillizer darts, dodged his bullets for a while, until I had enough of this, then I went down to him and sniped the Luminous Path member inside of the compound.
I then went down (max speed AUG, so this was possible without dying), and talked with Gordon Quick, who - to my great surprise - told me that he would kill me now for taking out "that boy." I concluded that he was talking about Louis Pan (whom I hadn't harmed in any way!) Looking around a bit, it seemed that Louis Pan was gone indeed.
So, I decided to have a look if the Luminous Path member inside of the compound was still dead, and climbed in by the roof. As you all know, they <i>really</i> don't like the player doing that... so I killed them all like the one in the courtyard that I had sniped from the skyscraper's roof. Then I looked at the exchanged keypad inside, and went out by the front door.
Gordon Quick was now VERY upset. In a quite different voice, he told me that he was going to kill me now, and then he began to chase me all over the market, killed the bots and all the police on the way (including some civilians who went in his way.)
When he refused to calm down again after several minutes, I decided that this was doing no good and reloaded.
