A good proportion of Italian cinema was made as cheap, disposable entertainment for domestic consumption. Just look at how incoherent a lot of the highly regarded films by sacred cow cult filmmakers are, Fulci's Zombie Flesh Eaters anyone?
P.S. No offense is meant by any generalisations, they're solely used to hypothesize a possible reason behind the OP's opinion.

I have not seen that film,although I head speaking about. Yes, here in Italy we are full of sh*t in the cinema, and there's alot of recommendations, and the actors were not talented, as in the infinite series of "Cine Panettone"... this is disgusting boys, and I am the first to be disgusted, although italian.
Exception done for some Dario Argento's movie.

No, they aren't even the most brutal in their genre (I believe Hostel has them beat in that respect). And as far as inventive ways to die, I think Cube wins in that respect as well, and is an infinitely more interesting movie to watch.

Certain scenes are properly a must see!! A katharsis in my opinion, like when the motel rapist get brutally killed. More than deserved end. Or that one with the acidhead who killed the John's son... knives on rough's face :ebil:
