Copy over just ddfix.ini (not ddfix.dll) to your t2x sub-directory.

Should D:\T2 be the folder in which Thief2 has been installed? For me that's E:\Thief 2\

Yes, usually this is the folder where you installed
Thief 2. Unless you have more than one installation (e.g., one for
Thief 2 and one for a
Thief 2 that is also used for the T2X expansion) on your computer, you can just substitute D:\T2 with E:\Thief 2 in the configuration file(s).
