What does choosing this difficulty level do specifically? I always play on Hard, but this run through (#18!), I'm doing it on Realistic. Seems I heard a while back that Hard was actually harder than Realistic because although the AI could take you down easier, you too had that power over them, so for an experienced DXer (#18!), it made the game too easy.
Any thoughts on this?

I've never played it on Realistic, but apparently it does just that, make the game seem more real, one shot to the head=dead, 2-3 to the torso? Dead.
Thats what I call hard.

In Invisible War, I think Hard is harder than realistic: on hard, you take loads of damage, enemies don't. On realistic, you take a SHITLOAD of damage, but so do enemies. Double tap to the head with the pistol is enough to bring down just about anybody short of illuminati and armoured templars.
DX, on the other hand, has a far simpler system. The difficulty increases the amount of damage you take. That's it. That's ALL it does.
And not only that, it's ONLY autoshot and shot damage types that it increases, too, so that's standard bullet weapons (pistols, rifles, shotguns), and autoturrets.
Flamethrowers (for example) are just as deadly to you on easy as they are on realistic.
And if you want actual numbers: the difficulty setting multipliers are:
Easy: 1.0 * damage (so no change)
Normal: 1.5 * damage
Hard: 2 * damage
Realistic: 4 * damage
And that's the ONLY difference.
And yes, since headshots count for 2x damage anyway, getting a pistol shot in the head on realistic will kill you instantly. 2 x 4 x 14 = 112, and you only have 100 health there.

Everybody around seems to recommend DXIW on realistic for at least SOME challenge.

No. The only two versions I've ever played of DXIW were the demo (1.0) and a lame Russian pirate (which doesn't accept patches and thus 1.0) and damage was defenitely increased against enemies on realistic.

After playing a bit this weekend on Realistic, (Just made it to Hong Kong) I've got to say that yes, the enemy damage inflicted upon you is much greater than at any lower skill level. I was getting dropped by one or two shots. It's made the game much more challenging, although the regeneration aug still makes the game too easy.
Anybody out their avoid this augmentation? I never have the willpower to skip it.
Anyway, my verdict is that Realistic is indeed the way to go for a tougher experience.

Having played DX1 on realistic two times, I have no doubts it's harder. headshots are especially lethal for the player, even from pistols at close range.
I pick up regeneration only when I specialize on firearms, that involves getting into firefights. There wasn't need for it when I played a silent assassin, in that case I was spending more exp. points on medicine skill.
