I haven't experimented, but your health does go down periodically while walking on lava. No icon showing the fire damage appears though (I do recall seeing that one at one point or another, but that may be when I touched lava or when I was too near a fire, I am not sure right now).
Which one hurts the player more, walking on lava or being sunken into it?

Not surprising. In Morrowind you took direct scripted health damage from lava, bypassing any resistances. I suspect it's the same here.
