From my point of view, those missions are bugs if you get them without actually asking for them. But at least one gets decent payment.

I really appreciate that the enemies has different positions when you reload a game. I've used the quick save many times just before being killed.. In another game i'd be out of luck, i would have to revert to a old autosave, here i dont :)
I'm still quite sure its intentional, to prevent things like that to happen and to make the game a bit more challenging, to make the experience a little different after each reload.

Yes, but still odd that only friends change their positions for him, in my game i have noticed it with both friends and foes.

That actually only seems to happen with the friends. They sit down by a fire, I save, then load, and suddenly they are standing and preparing to sit down; like I went back in time.

It seems the game just doesn't save state/alertness of NPCs - the parameters are re-initialized every time a saved game is loaded.

Interesting, how many Souls you found in the game?
I have 2 of them now and when both are equipped they regenerate health quite quickly, so I thought what if I had 5 of them equipped? That pretty much should make one indestructible, though I doubt there is more than 3 of them in the game. I had 2 when I finished the game for the first time.
