So I hear it's a really good idea to include game elements that fit your narrative. Give the player a HUD and Radar in a science fiction game like System Shock or Deus Ex and it makes a sort of sense. It's not a big stretch sitting here in the now to imagine that kind of thing in the future.
So we know they've fictionalised NuGarrett's Focus ability. We know it involves an incident at a Brothel/Opium Den. But I'm really really curious as to what could happen to a chap in a brothel/opium den that would equip him a medieval HUD and Radar.
So lets have some wild spoilerific gamblin' fun. He's a bit pasty looking. We know The Crow is an influence.
Is NuGarrett...dumdumdummmm.... dead?

I thought it was common knowledge that one of the side effects of opium addiction was sonar.
...though I will admit that the HUD does take things a little too far. Oh well, I'll chalk that off to suspension of disbelief.

The entire game is actually a parable describing drug addiction, where a bleak Garrett's final toke is one too many. He hallucinates some of his thieving exploits, as each mission becomes a further stage of descent towards his inevitable death by overdose.
In a fourth wall-breaking, ironic turn of events, Garrett's demise is also revealed to be a metaphor for Square Enix/Eidos Montreal's treatment of the beloved game series.

I found a short documentary of Garrett out of costume (it's like seeing KISS without the makeup for the first time!) that explains how the shroud works:
