Are "?" items in NPC inventories totally unpickable, or can they be picked with extremely high stealth? Example: King Lanshire's two keys, one to his room and the other to the gate leading outside of Arx, and Gary's bank key. I've tried stealing them with 120 stealth and they're still only question marks in their inventories. I've read old posts where people said they could pickpocket these items. Was this something that was changed with the patches?

You definitely can't do it with the latest patch, and I'm not sure if it could ever be done. The older posts are rather ambiguous.
However, it is possible for a skilled character to assasinate the King and a few of his subjects and get away with it without setting off the "telepathic alarm" that alerts the rest of the people in the kingdom. Obviously, if this is done early in the game you won't see some cutscenes, but it's possible to complete the game even if you butcher everything that moves.
