It's not a fake, but really a leaked internal alpha or beta of official patch/mod 1.1 (confirmed by insider). Some files maybe from some fan mods, but devs have agreement and permission of authors of all used files.This version is from October-November, further work on the patch has been suspended until release of CS. Now I don't know if it works with English version but it does work with Russian version 1.0004 and doesn't work with 1.0005. I haven't yet checked it myself but comments are here in Russian:

Generally, plenty of changes. I suppose the changes are intended for "free play" mode only because script characters are gone, some other features:
game became considerably more difficult,
trade balance is improved,
reputation and ranks work,
more monsters, lesser people,
A-life is turned on (to a degree, but is manifesting itself much more than in original game mode),
anomalies aren't visible, even electra,
artifacts are very sparse, but cost much more,
radiation effect doesn't go off by itself,
bandages and food don't heal,
there are some "horizont backgrounds" in a folder of the patch but they apparently haven't yet been implemented in the game,

Boooo on invisible anomalies. Those things provide the most interesting visual effects in the entire game.

I just tried it a bit. To install it just unpack it to gamedata folder.
First observations:
There are plenty of marks on the map, but descriptions are just some code, no text, though it's not hard to determine what is what
Sid wants money to improve my reputation :)
Limited stock he is offering is quite costly
Stalkers in the village refuse to talk with me, but one of them told that they suspect everyone who is delivered by Death Truck and won't talk until I earn certain reputation
It's very hard from the start even on easiest difficulty, a bullet or few is enough to kill you, so basically it's hiding and running away from every tiny suspicious sound for now :)
The Cordon map is enveloped in a semi-thick fog when you start the game

I guess they will stay visible in the "story" game mode, they removed it only for "freeplay" mode. I read that after player gets a certain reputation, he/she may contact a scientist trader who will have additional detectors for sale.

Does the few bullet kill applies both ways? We finally got official realistic weapons mod?
And fog is bad, I always liked Cordon because it felt so safe and comely :(

Invisible anomalies = No 1.1 alpha on my end
And it'd be stupid if the increased bullet damage did not work both ways. :erm:

I don't know yet if that works both ways or not, but one thing for sure - one bullet from a pistol you have in your inventory from the start vs. one bullet from an assault rifle that a guy I encountered just near the village had definitely weren't the same ;)
