It could also be a mix-up of some of the missions of frobber's "A Keeper of the Prophecies" campaign. The part with potions against a deadly disease (or rather poison) is in the second mission, "Hallucinations". The ghost of a deceased woman from the first mission guides you around. The cathedral, however, is located in missions 3 ("The Insurrection") and 5 ("The Other Side of Time"). The campaign is for Thief 2, although the first mission was published for Thief 1 earlier as well.

sounds like he is talking about the mission, where you have to get to the church in a time limit meet the guy and then the alarm goes off and you have to get back to your place to get gear, the girl is in your house, of course i cant remember the name of mission but it was classic at the end of the series you and the girl head back to her place and get ambushed by some archer, beat him and walk in her place for mission series to end
also in earlier parts you can find certain items to combine in a pentagram place, one object is to throw chalice in to a forge, i am sure that will help with naming mission

I think downwinder may be thinking of Calandra's Cistern. But the description to me sounds like Keeper of the prophecies as well.

Downwinder's FM idea sounds like the second mission of Calendra's Legacy, possibly mixed up with Calendra's Cistern. But there's nothing remotely like being poisoned or sick in these missions, as far as I remember.

Actually yes, reading this again, it definitely sounds like KotP. The only thing I don’t remember is the zombie/plague thing. But everything else described is in that campaign.

YES! This sounds correct and the screenshots for it are definitely pinging my memories. I suspect I might have grabbed individual missions out of order back then. In fact there is a mission called Hallucinations in my files I salvaged from my ancient pc from back then but for some reason I'd eliminated it as a possibility. I just loaded it up and yeah, this is it! Thank you so much, I really was starting to think I'd imagined it.
Edit: I also wanted to say that the fact there are so many fans of this series who are so dedicated to preserving it and making new content and helping other players means so much to me. This was my favorite game as a kid (and still is) and got me interested in going into game dev and it's heartwarming to see that it's still so beloved. So thank you all again.

i mis took poisoned with the time limit to get to church in time yeah it was calandra series
but kotp is brutal series, that planet with monsters in caves
seems like some missions are hits and some misses

Well downwinder that mission you mention is bugged too. That makes it even harder. If you hit the one crystal early it will be game over. But I still love playing it.
