The first missions in my mind are the Den which is the 4th part of the Larsene series and Malazars Inscrutable Tower by nicked.

I know both those and it is neither , I believe the overall level is more of a down in the bonehoard type level, with crypts to different families.

The 2nd mission of "The Tomb of St. Tennor" contains a family tomb/crypt where you can steal a golden skull, and then some statues in this part of the mission turn into golden guards. This mission is definitely a "Bonehoard look-alike" one.
And while we're at it... The mission described by Gerhard Eichberger reminds me of the first version of Cardia's "Citadel of Douro". A "somewhat townish" area with many walls that form barriers in corridor-like streets formed a labyrinth. Mechanists were around. A later version of this mission, made for NewDark, did not have these walls anymore, which was good - they were a great nuisance without any apparent reason, if you ask me.

What was the mission that had a custom CTRL+ALT+SHIFT+END level with a small interrogation room? It had a large rope/chain across the level with a keeper squatting on the rooftops watching over you.

This sounds like one of GORT's campaigns. A fairly recent one, for a contest. Could be "TPOAIR" ("The Pursuance of an Inscrutable Reciprocity"?). The first mission reacted like this if you used CTRL-ALT-SHIFT-END. You landed in a small room with a keeper-like figure behind a desk, and said something like "Cheating? Seriously?".
