Does the PC's gender make much difference?
I've only played as female Alex and the only gender specific effect I noticed was Sid Black offering a discount.

I've played both genders. It does not affect the big picture in any huge way. A few exceptions are there, though (ie Sid Black will discount his price if Alex is female; also what pride mentioned).
I find the genders give unique dimensions to the story in any case.
Also Chris Sabat and Laura Bailey play the voices of Alex, so I give points there regardless.

I just noticed that NG Resonance's manager will say "I thought you were that Pequod's guy again" even if you are female (I know the light is bad in the arcology but c'mon!).

I never even got through the first level as male Alex; I felt like I was piloting a flaming homosexual. He sounded too . . . feminine. I don't think I could handle an entire game with him. Give me female Alex any day of the week.

I played IW twice, once on each gender.
it was still crap lolol It just seemed like another missed opportunity for real depth.
