Mods are great for this, try (
http://www.strangebedfellows.de/index.php/topic,542.0.html) Pop's Difficulty and Enhancement mod. If that's not enough, try doing it on impossible with no cyber modules or gandhi mode or no melee weapons or whatever other crazy modes people can come up with.

"Backward compatibility"
You have to walk backwards (slow, and AGI helps). You can still pick up stuff on the table by turning around.
"Backward incompatibility"
Which means you won't pick up any items from tables and corners. If you have to pick up items, try PSI-pulling or whatever techniques you can think of.
"Left Alone on a Space Ship"
You have to run forward and always turn left at crossings, as much as possible.
"Monster Magnet"
Attract as many monsters as you can. Let them follow you everywhere.
"SHODAN's Child"
Only use the Crystal Shard, and do not kill SHODAN at the end. (The ending sucks anyway...)

Gather a few multiplayers and tackle the most impossible runs.
"XERXES Becomes Reality"
SHODAN may be in her physical form on Deck 9, but you are XERXES during the entire game! The goal is to build Turrets (cheatcode) wisely. Try building five Turrets in an area to kill 50 enemies.
"Rumbler vs MP Robot"
Play through 6 decks just to see the epic "Rumbler vs MP Robot" on Rickenbacker. Build two types of turrets and see which big guy will die first!

Crystal shard is an epic weapon. I essentially used it exclusively for the last third of the game, except the assault rifle against SHODAN.

Anyone ever try a Tech Specialty run leading to getting Exotics maxed out as much as possible w/subsequent emphasis on using the Viral Proliferator & Worm Launcher? Considering how late in the game they arise, I'm guessing it would be, for the most part, a pretty wrenching experience.
