It is perfectly reasonable to give the next game in a franchise the informal title of '[IP title] [installment number]', where the '[installment number]' isn't especially referring to anything other than the fact that this will be the nth installment of the IP released.
Anyway, all I would really like to see for Thief 5 - i.e. the fifth released discrete title in the Thief franchise - is that it exists at all. (Although I would like to see the original voice actors return, and the game to have a tone and playstyle closer to that of the originals.)

Pedants! Wha bloody difference does it make?
Deadly Shadows is the third game so it can naturally be called Thief 3. Thief (2014) is the 4th game, so why not call it Thief 4 for clarity?
Anyway, Thief 5
As we know, Erin did not die at the end of Thief 4. I'd like to see her back with a faction of her own, unknown to Garrett. Give the player the option to play as either character.
Instead of having linear progress, make the game more open with multiple routes to take and multiple endings. Taking different directions and making different choices in the story opens different chapters.
I like the idea of having specialist tools, as in Thief 4. Maybe this could be expanded on with more specialised tools that are required to progress.
The lockpicking mechanics should stay as they are from Thief 4.
Make more of the map accessible, there's nothing more annoying than seeing a room that cannot be accessed.
Make the map more open world, like WoW. Maybe travel to other towns and cities.
We've already got tame frogbeasts. Make it possible to buy and train burricks - ride them to remote areas, use them as diversions.

The series ended with Thief II: The Metal Age; however, T2X could be considered an unofficial T3.

Witches could be new faction along pagans. They could oppose the clergy in the city. Necromancers and mages could return. I think Keeper Enforcers are very interesting group to explore.

Lots of possibilities. A para-pagan witch coven could definitely work. Alternatively, some brand new faction. Necromancers (like those proposed for (

If you wanted to be more conservative, you could center the game around a resurgence of the Hammerites, coinciding with the fall of the Mechanists (I'm sticking to T1-2 continuity here, since T3 threw a monkey wrench into the lore).
The Hammers regain their former glory, influence, and control over the city. The Baron restores their status as law enforcers, and they reimpose theocratic rule over the City, cracking down on other factions. Pagans living in the City are given the choice to convert, leave town, or be jailed indefinitely. Book burnings become common. Garrett's mission is to undermine the Hammers, discredit them, and ultimately tear down their structure through a variety of means.
