Odd, I have no idea why that would happen....I mean Dagoth Ur almost always takes a header into the lava pit, but that shouldn't mess up the ending.

One common malady w/ the game not recognising that you killed Dagoth Ur is if you recall or intervention out before talking with Azura. (Once you kill him, she's waiting at the exit of the 1st room you saw DU at) Not sure if that's the case here, but it's a possibility.
