So I suppose the perfect thieving would be not only first-class ghosting, but also operating like the real Garrett would, with his calculated steps, his head movements, his aim, his thoughts, and of course everything done within a single shot, no saves, and a sole misstep would be enough to restart the mission. It'd be the Mamma-jamma mode.

Perfect Thief is achieved if abiding by the official Ghost rules while also getting all the loot. No saves is Ironman.

But the other points? How many times did you run when the real Garrett would walk or creep? How many times did your marksmanship fail when the thief would readily hit the mark? Or how about staring at Cutty or Raoul as if you were listening to them (besides other body language hints)? When such fixation arrives, then it is Mamma-jamma mode.

Not sure what you're getting at here. To use your logic, the "real Garrett" you speak of doesn't exist; he's a character in a video game, played by a human. So in essence the player is Garrett, and therefore whatever the player does, Garrett does. If I miss the mark, Garrett does.
Let's get the discussion back on point here. I thought since you asked the initial question you were interested in hearing the community's opinion.

"No detections or physical contact" is not really a loose definition. Dishonored and Deus Ex are just misusing a term. For them it means something else because it's basically a Thief-specific term.

I think staying undetected is the #1 rule, and it's nearly impossible to escape unseen after the alarm starts. That's why the level is so hard. Escape and Haunted Catheral are the same. There are so many patrols that are difficult or impossible to avoid entirely.
Maybe I should have included Bonehoard as well, since you need to use fire arrows and alert zombies to get the Soul.
