(Yeah, I saw the original film before it was called "A New Hope" and always figured the whole "Episode IV" thing was a gimmick that was never meant to be taken seriously.)

That Deleted Magic DVD that RBJ mentioned was fascinating, not only for the deleted scenes but the alternate versions of existing scenes. The first cut of the Death Star battle was hilariously dull; it's remarkable the effect good editing has on good film.
Also I don't think anyone's got the "Episode IV" thing quite right. "Star Wars" was originally released in 1977 where the opening crawl just said "Star Wars". The movie was rereleased in theatres in 1979, at which point Empire was in production, so "Episode IV - A New Hope" had been added to the opening crawl. Empire then came out in 1980.
It's that first rerelease in 1979 that, I think, is the reason why so many of your parents remember Episode IV being around since the beginning. It was definitely not there on the '77 print, but the '79 print was before Empire so people remember it being "before Empire" and therefore "always there".

On this note, its useful to know (so Lucas has said in interviews) that the rough inspiration for the story was a Japanese movie called The Fortress ... with samurai = jedi (at least the kendo style sword fighting didn't change much); master/apprentice = obiwan/luke; the castle = deathstar; princess = princess, same rescue mission; 2 peasants = 2 droids ... and I think that's about as far as the parallels go, the rest is Lucas and a few homages to other stuff.
Lucas wrote the much longer story arc later which put the movies in order, and you can see the results. E.g., you can tell that Empire & Return of the Jedi were written as a set piece together, and you can tell that Episodes I-III were derived from really big but sort of hazy ideas that had to be there to set up Ep. IV, Anikan's childhood and being picked up by obiwan, the clone war, etc...

Yup, Lucas has said that there were no plans for a sequel at all when they did the first movie.
Mark Hamill had said that when he found out that this might be big and they stated talk about doing a sequel, he tried to get out of his television contract (Eight Is Enough) and failed. He hadn't been planning on sequels either. (I believe it was a motorcycle accident that got him out of the contract and freed him up to do more Star Wars)
