I can't even tell what the visage is based on, what instructions or sketches led to those choices. At least it was only that one time...as far as we know...and thankfully, only concept art.

I agree about fire elementals (they really slipped my mind), but don't remember really any fireballs thrown by mages. Well, maybe because I never really fought them and it was only one mission. Projectiles shot by traps never really looked like firaballs to me though (more like some kind of energy bolts), neither did the attacks of priests.
When I read that quote, I thought they were talking about fire arrows, that's why I was surprised.

Overmind, I think they were possibly referring to the Fire Elementals (which also shot fireballs), actually.
Yeah, there were the purple magic missiles that looked like purple diamonds, shot by proximity traps and pressure-plate traps in the Bonehoard, proximity traps in Constantine's mansion, and from red Spider Beasts.
There were the fireballs, with one emitter in the Bonehaord that regularly shot one at a distant wall (great for luring undead to get killed by it), but otherwise shot by Fire Elementals in Karath Din, the Maw-like portions of Con's mansion, and the Maw itself in TDP/Gold only, and fired by Fire Mages if you were seen.
Tiltowaits were made, and are still available in DromEd, but never made it into the game, as their explosion would generally crash the engine. They looked like torch flames, but if you got close enough, they'd fly at you and explode!
We saw several Mystic's shops in the games, though Carmen Cantata in TDS is the only mystic we interact with, yet her business isn't related to it.
