That's promising. Also check DirectX Graphics Adapters / ... / Display Modes. The pixel format is X8R8G8B8.
So what's the module and offset of the crash?

5040x1050 is also listed in directx format X8R8G8B8. The error listed in the errorlog is :
shock2 caused an Access Violation in module shock2.exe at 0023:00517afe.
I wasn't sure if that was what you were looking for, though.

I run my 5870 in eyefinity all the time.
I actually just started a new playthrough Battlezone 1998 in 5040 x 1050. If the widescreen fix could work with SS2 i'd definitely play it. It sounds gimmicky but once you see it in person it just seems awesome.

You're probably hitting some limitation in the game code, which is bad news. What's the biggest resolution that works?

1680x1050 is the highest I have seen work. I have also tried 2400x600, 3840x800, and 3360x1050.

I wonder if 2048 is some kind of limit, then.
If the limitation is in the game code, one possibility would be to have ddfix use a bigger screen than what shock thinks it's using. I'm sorry to say I don't know when, if ever, I'll have time to look into this.
