Okay, let me explain it to you like you're a 6 year old...
MMOs are a genre. It's designed to be such from the ground up. If only 500 people end up playing a game, it's no less an MMO than another game that has 5 billion playing.
The amount of subscribers is only indicative of its popularity. Being more popular than the games that came before it doesn't make it the first in its genre.
So with this in mind, we can say that UO was the first break-out successful MMO, but not the first MMO ever.
