Weren't you the one who played DXMD on a very old card? I remember somebody posting about that.
Anyway D2 does 15-25 fps on my card with everything on very low (textures medium) at 720p(100%). It's good enough for me, especially since I play stealthily. And apart from the textures, it doesn't even look bad. :)

Yeah, that was me. It didn't look stellar but it ran smoothly enough.
15-25 seems a bit low for a game with such fluid movement. I'd be willing to give it a go, though.

Hotfix Nvidia driver + DH2 beta and still getting massive screen tearing. And apparently that's something impossible to show in screenshots because any screenshot I take looks fine.

I solved my tearing problems, and I think it's related to how the 'visuals' tab doesn't seem to save settings properly (the adaptive resolution setting resets to 75 everytime you relaunch the game).
When I first launch the game, if I go to the tutorial I get tearing immediately, regardless of graphics settings.
If I go to the 'visuals' tab and use the option to reset everything to defaults after I launch the game (either from the main menu or in-mission), I can then set those options to anything I want and don't get tearing.
Next time I launch the game, regardless of what it shows as those settings, I get tearing until do the 'reset to default' thing.
It would appear to me that those graphics options aren't being saved correctly. I'll have to check and see if it's somehow related to the fact that I run Steam as administrator.

What card do you have again?
The reason I asked is because there must be some weird setting deep within the Nvidia control panel you've flipped on or off, because I'm not seeing any tearing whatsoever. My only guess as to why is because I haven't screwed around with my GPU settings all that much, so...hell, I dunno.

GTX 970. And I've fully cleaned out the drivers and reinstalled them with default settings then run the GFE again (Win7 64-bit). Obviously I play tons of games (or try to) and I haven't had a vsync issue anywhere else.
Are you also getting the bug where it doesn't save the Adaptive Resolution setting?

Something else interesting, looking at my dishonored2Config.cfg file, is that there don't appear to be lines for vsync (I forget what IdTech uses for that) or adaptive resolution, and those appear to be the two specific settings that the game isn't saving.

I haven't messed with the adaptive resolution settings, so unfortunately, no.
Though I did check out my .cfg file to see if maybe I could find a setting for vsync buried somewhere in it, but I'm turning up the same thing.
On a random off the wall guess, do you have a monitor that supports 120Hz+ refresh rates? There could be an issue with the engine not handling vsync well on anything above 60hz. You could try and force it down, and see how it acts.

Nah, I have the standard 1080P 60hz monitor. Could be that my drivers default to vsync off unless the application overrides, and the application isn't saving/loading the setting right for some reason. Tried running Steam out of administrator mode and it made no difference. Might try deleting my cfg file and seeing if it makes a difference.
Edit: Nope, same shit.
Edit 2: The 'reset to defaults' thing only seems to work if I change a setting first so it has something to reset back to (i.e. 'restore defaults' doesn't fix it if it's already set to the defaults). Also, I thought maybe there was a specific setting GFE was applying that was making it weird, but no, if I do the 'reset to defaults' trick then change everything to match the settings GFE uses I get no tearing.

It wouldn't hurt. Also, go through the Nvidia control panel with a fine toothed comb if you have to. I can't think of any other reason why it works for me, but not for you, when we both have about the same card.
Oh, for shits and giggles, run the Geforce Experience, and optimize the game. I don't see any settings for vsync in there on mine, but what do you have to lose?
