What exactly is altered when you choose the different difficulty settings? I've only played on Easy and Medium before. (Yes, I'm a wimp. :p) I only really noticed that damage to me is increased, and maybe better accuracy on the AI's part. Is there anything else?

No, nothing else changes. They even say so in the booklet.

Oh... don't have that with me. I'll have to take a look at it next time when I get the chance. Thanks though.

i played it on easy as expairemental mode and play realistic mode, i tried hard, all the settings r the same 2 me

I have heard that in realistic mode hit location are (better) computed and you get more realistic results if you-e.g.- put a bullet through the head of a person, like one shot = dead.
I have only played on the realistic mode, though. So I take my advice from a friend who played both easy and realistic.
I dunno, but if you get shot in the legs in easy, do you also become slower and lie on the floor. Or means realistic only that the enemie gets better but is in some instances easier to kill?

Damage you do to everyone else is calculated the same regardless of difficulty level. Setting a higher difficulty level increases damage dealt to you. Simple as that.

On Realistic enemy guards are tougher than you are. In the cathedral I snuck up behind an enemy sniper and shot him in the back of the head with my rifle (advanced skill). He whirled around and shot me in the chest with his rifle, and I died instantly (from full health). This happened twice.
I guess it's because he's got a helmet and I don't have body armor.

I've had certain guards turn around and kill me after I shot them straight through the head from behind with master rifle skill (even though I was cloaked, stationary, and under 20 feet away). Don't think it's a matter of difficulty setting - I've never played on easy but I seen it happen on hard and realistic. I don't know whether it's a bug or whether certain guards on some maps have been given extra protection by design.
The Cathedral is where I recall the most incidents - the snipers on the overpass walkway approaching the Cathedral and the snipers in the towers.

Blah, those Cathedral snipers are annoying... Though I've never actually gotten shot by them whilst outside [I should try other routes of approach, the current one I use seems to work too well *g*], usually only when I kill their buddy [when inside]; they snipe my head off 2 seconds later. Luckily they don't really care about corpses around them as long as you hide right away and go back to standing around unalerted. Go fig. :p

I gave up on Realistic once I reached Hong Kong, with the first encounter with an MJ12 Trooper (the big bastard ones). I managed okay until I was exiting the Lucky Money club, and you get "ambushed" by one of them. No matter what tactic I employed, he always killed me. Shame really - though i'm sure i'll try it again some day. Any tips apart from trial and error?
