Well often underground bits have two exits, the way you came in, and one close to where you complete your objective/find loot

nope...completely different...and as I said on my map it points to it as the exit and says "exit to mountains" and my guide says it is the exit.....but all I can find in the spot is ceilintg, floor, and wall. I've tried standing in different places hoping to get the "open door" message etc....its weird...I've had problems finding trap doors before but this is amazing. Have any of you completed this quest?

Just try loading a saved game before you entered the joint. I've done the quest myself, and got out fine

I save as I go through the missions...don't have any previous saves unfortunately.

So you would be in Sancre Tor then?
Think about it, was the entrance there (the wall on the stairway) when you came in to the dungeon? No it wasn't. The entrance/exit was at the end of a tunnel in the top "floor" of the dungeon.
Just like ercles said, you are at the right place, but on the wrong "floor". You need to find the same place in the floor above you.
If, for some reason, the arrow points to completely wrong place, just follow your route all the way back up and screw the arrow. (sometimes the quest arrows don't work quite right)
And, for everybody's sake, keep more than one savegame at any time. There is no reason not to have multiple saves (and save often).

I'd advise doing a backup save in addition to your regular one every time you enter a new "dungeon" or start a new quest. Even if you do everything right, saves can become corrupted on occasion.
