so where do small-medium businesses with minimal management costs fall?
I think something we can agree on is that excessive bureaucracy is something that undermines most sectors of the american economy and government, and those institutions that minimize management costs generally provide a better experience for the customer. Its not a blanket example, but in my experience privately owned and small-chains (for example, one fast food joint that operates several locations in savannah) restaurants generally provide much better service than chain restaurants.
On the other hand, I can think of two other explanations. The first is the competitive factor: if they don't, there's no reason to choose them over the consistent quality a chain provides.
A second reason not directly related to management is the disconnect between provider and customer. With most (note MOST) chain restaurants, the manager has little reason to provide good service. As long as his restaurant remains clean, and no complaints are filed, his compensation remains consistent. For the manager who owns the restaurant, every customer who doesn't want to come back means he loses money.
I've rambled a bit, but my feelings on the matter is that those who obsess over the virtues of "capitalism" (ie, sean hannity) are being intellectually blind, as they ignore the negative effects on individual freedom that "mega capitalism" has.

Problem is, consumers won't start spending until they have money in their pockets and feel reasonably secure. But they don't have the money, and it's hard to see where it will come from. They can't borrow. Their homes are worth a fraction of what they were before, so say goodbye to home equity loans and refinancings. One out of ten home owners is under water -- owing more on their homes than their homes are worth. Unemployment continues to rise, and number of hours at work continues to drop. Those who can are saving. Those who can't are hunkering down, as they must.
Eventually consumers will replace cars and appliances and other stuff that wears out, but a recovery can't be built on replacements. Don't expect businesses to invest much more without lots of consumers hankering after lots of new stuff. And don't rely on exports. The global economy is contracting.
My prediction, then? Not a V, not a U. But an X. This economy can't get back on track because the track we were on for years -- featuring flat or declining median wages, mounting consumer debt, and widening insecurity, not to mention increasing carbon in the atmosphere -- simply cannot be sustained.
The X marks a brand new track -- a new economy. What will it look like? Nobody knows. All we know is the current economy can't "recover" because it can't go back to where it was before the crash. So instead of asking when the recovery will start, we should be asking when and how the new economy will begin. More on this to come.

And how is not related to employment? You say that the bad employment rates don't matter because there's plenty of other statistics to consider, and he says that

Thats why the health insurance problem is so messed up, it requires enough money to start up that its restricted to the American business elite, who amply demonstrate that they don't have people's best interests in mind.
And, again, don't make the mistake of assuming (because conventional wisdom assumes so) that government and business interests always compete, its why the restriction on price negotiations was put into medicare and why attempts to fix it have been repeatedly shot down (by both parties). There's a good USA today article on it that was posted last year.

Actually I have dealt with all three for the past three years. My power company (Florida Power and Lighting) and my cable, internet, and phone company (Brighthouse Networks) are a huge part of the fuel that drives my passion to eliminate as many federally run bullshit programs as possible (did I forget to mention that pesky $1.8 trillion 2009 deficit?). These companies have displayed exemplary service through hurricanes and wildfires. Why the hell would you think a 22 yr old wouldn't have dealt with a cable company?
