This happened to me before with the 6th Ash Vampire. Now, after beating Dagoth Ur for the first time, I try entering Akulakhan's Chamber and my game shuts down each time. And I can't even use player->coc "Akulakan's Chamber" because it'll still shut down. I guess I am screwed forever right??

As odd as it'll sound, I suspect you may just have a graphics issue at this point; have you tried updating drivers, et. al.?
