1) Just run towards the end of the loooong corridor to your left when exiting the elevator. Now you will see two shotgun hybrids enter the corridor, in one of those rooms is a Toxin-A.
2 and 3) To your right after exiting the elevator is a door, just follow the path until you come to a short corridor with many doors, one of them being broken. (a shotgun hybrid is famous for the ability to walk right through it)
Enter the door next to the broken one and you will see a broken replicator. In this room is a window, break it and jump through it. In this room are two Toxin-A vials if I remember correctly.
4) On the floor in the first room of Hydro A, lies is a corpse that has a vial of Toxin-A on it.
This is all you need but there is a 5th vial of Toxin-A, I suggest that you don't put it into your inventory, just leave it alone.

Why? Does it sometimes bug out the game or something?
My last playthrough that I just finished I carried around all 5 at the same time before I used any of them, to no ill effect.

I never experienced any problems with the 5th vial of Toxin A. But there have been some posts about it causing a bug when changing levels. Most I found were only referencing hear-say. The earliest description of someone who actually experienced something of a Toxin A bug was (

That makes me feel better, but I won't take any chances. I don't need a 5th Toxin-A vial anyways.

I always carry all five around with me until I notice I have the extra one and throw it out, usually a while after using them all. I've never had any problems.

Even though taking the chance would be a surefire way to permanently either confirm or dispel your conjectures regarding the Toxin A, you prefer to carefully avoid a problem that may not exist.
If that's common sense it's boring.

Confirming my theory as false or true; it would still mean I only carry 4 Toxin-A since there are 4 environmental regulators. So I see it as nothing but a waste of inventory space to carry the 5th vial.
But it would be interesting to try this out, can't deny that. Too bad Deus Ex gets all my attention these days. ;)
